SINGAPORE - On April 6, operations manager Samuel Tan received a call that two elderly women had fallen while using an escalator at Bedok Mall.

The 34-year-old, who handles day-to-day operations at the mall, including matters about security and cleanliness, had been in the job for less than a week.
