A review by the KK Women's and Children's Hospital (KKH) into how two babies were swopped last month has found poor system checks in place.

Staff were also not following procedures, according to the findings made known by the hospital on Wednesday.

Six nurses have been given verbal or written warnings for their carelessness.

Health Minister Gan Kim Yong had directed staff at the Ministry of Health to find out how the baby mix-up could have happened last month.
