Heart probe recommended was not appropriate

MY SISTER accompanied my father to the National Heart Centre on Oct 22 to inform his doctor he would go for a procedure to determine the size of the hole in his heart, as recommended at his last appointment.
After the forms for the procedure were filled up and only when my sister asked if it would affect his other medical conditions did the doctor realise the procedure was unsafe for him. An alternative procedure was then recommended.
When my sister tried to find out more about it - the risk involved and whether my father needs to stop taking warfarin - she was interrupted by phone calls for the doctor. The questions were not answered and the consultation ended abruptly after the doctor showed some hand signals and a nurse rushed them out. My father and sister were called into the consultation room later, but another patient was present. Although the doctor asked only about the medication my father is taking, they felt uneasy. Patient privacy is important and should be respected.
I have the following concerns:
Do doctors at the National Heart Centre have access to the online Electronic Medical Record System or make use of it to check a patient's records? It may be difficult for some patients to inform doctors of their other medical conditions or the names of their medicines.
Do doctors maintain a checklist to assess a patient's medical conditions before determining whether he is suitable for a recommended procedure?
What are the checks involved to ensure that appropriate medical procedures/interventions are recommended and correct medications are prescribed, should the patient give incorrect information or is not certain about his own medical condition or medication?
Are there designated doctors or nurses to advise patients and their families on medical procedures, besides giving out information leaflets? This will allow the families to ask questions so that they can make informed decisions.
